- In some rare cases, there will be a malfunction in the Holodeck causing massive damge to the ship or kill off crew. These will only happen when the holodeck is needed to increase happiness.
The holodeck has malfunctioned! An evil holodeck character has killed many members of the crew!

The holodeck has malfunctioned! Transdimensional energy beings have become attracted to your holodeck. Explosions rock the ship.
